THEY REFUSED TO LISTEN TO ME WHEN I TOLD THEM TO USE A PARTICULAR ADDRESS & PHONE NUMBER WHEN HANDLING THE CLAIM FOR LABS.

They were given an address and phone number to use when submitting the insurance claim for my blood work due to them being out of network. My insurance covers it but the address and phone number must be used. They didn't abide by my request and the claims are now in collections ruining my credit. User's recommendation: If you have special instructions don't rely on Quest to listen.
