I tried to contact ent and allergy associate by phone, it was impossible to reach them, after several attrempes when I finally reached them, they hang up .

They would not answer the phone an finally when they did they hang up on me. I needed a form to be filled by my insurance carrier. User’s recommendation: My problem was solved the next day after I contacted you. They sent me the form not even a day I contacted you..

After spending 85 minutes to speak to rep finally when she answered she hung up on me

On June 12, 2020, at 3:20 i had a visit in White Plains N.Y with audiologist repair and visit $225. I need the UB-92 or CMS-claim 1500 claim form filled and signed and emailed to me per my insurance co. Thanks
