Receipt for gold plan

Didnt get a receipt for the gold plan and it locked my card that i use and i need it it been 2 or 3 months i have not got someone to help me the last person said he was going to help and said he was going to get back to me but never got back to me but hos name was wade and never got back to me so iam emailing again
