In my order by phone I gave my father's name and middle initial, date of birth, and date and place of death. I don't remember now, but if asked I also gave his parents' namse. The death certificate that was sent to me had the wrong middle initial, the wrong birthdate and the wrong death date and different parents from my father's. It cost me over $30 and I am not at all happy with this service. A User's recommendation: don't order by phone.
