To whom it may concern: I am writing from Dr. Jon E Noll's office, myself, NVA and the patient have tried numerous times to reach you about canceling an authorization for Max Jones DOB 02/19/2010. I have also sent a request for a records release. Please promptly act on both of these. Todays date is 8/16/2021, Thank you sincerely, Jennifer Office Manager
