I finally had to report this company to the EBSA/Department of Labor. They sent the initial election forms with the incorrect information, lost my election form, letter and check even though it was sent with a tracking number. They sent me coupons for payment for months I didn't owe and after 35 days of dealing with their incompetence, my husband STILL doesn't have health coverage.
