I am a previous employee who needs proof of the flu shot I received last fall October 2017 for my new job. I have been asking my old manager to send this information to me and my new occupational health services so I may start my new position. She at first said she couldn't find the documentation and then said she sent it even though she didn't and is now ignoring my request completely for the last two weeks. I am unable to start my new job without this information And I have a right to my medical record. Very frustrating. I did also email the regional manager and another higher level manager and have not received any response as of yet.
