Received something

My father suddenly gave this thing to me from post office. I've played FreeLotto long time ago and I still remember that I won a $1. I saw a prompt if want a check or donate it, I wanna know if this is legit so I clicked Yes. Months passed and I received this weird thing. What will I do with this weird paper? Should I believe that this FreeLotto is legit?
