you can’t upload the form to get the 50.00 walmart card.

They promise you a 50.00 dollar Walmart card: if you order the veneer teeth they advertise on tv. So I place an order., then they offer the magazines to get the gift card. The teeth are horrible and look very fake. makes you look bucktooth. So as of 11/29/16 I have cancelled the magazine subscription and cancelled the order for the teeth on 11/27/16. I don't want the magazines at all!!! They better not bill me for anything, as I stated I already cancelled on 11/29/16. This is truly a RIPOFF!!! (SCAM)
