Upset about billing and no follow up to phone calls or email

I was recently billed $1,032.10 for my monthly policy ... it is suppose to be $ 222.30 monthly ... called several times about the error ... spent several times on hold for over 40 minutes ... talked to at least 7 different agents trying to resolve the issue ... sent several emails and received a follow up days later ... its now a week later ... i have no idea if i have insurance since i haven't been contacted by anyone with a positive resolution ... i've been in contact with a supervisor but have yet gotten any satisfaction over the last three days ! Soooo ... where do i go from here ? It seams that no one has any answers to my questions !!!
