Unable to resolve clerical mistakes – refusal to reimburse or cover meds

I have been trying to fix a mistake made on their part since December 27, 2018. They claimed I had another insurance though I did not. Thus, they denied coverage. I was on the phone a dozen times talked to many who could not figure out about mid January. Finally they admitted to the mistake. Guess what? Today I tried to get coverage for a med and was refused for the same reason. I talked to 3-4 different people and was on the line for over 30 minutes, AGAIN. They agrred there was a mistake, it suppposedly will take a few days to fix it. That is what they had told me 3-4 weeks ago. Also they made me fax forms for reimbursment. I called them to find out if they received the fax (that too was at least 10 days ago) again no one could tell if the fax was received. They claimed I could leave a voice mail to rate the conversations. The voice mail only asks about the clerk on the phone, but does not allow to leave a voice mail.
