Shipment cancellation

Hello, I would like to cancel future shipments of my Crepe Erase Kit. I am almost three full orders ahead on shipments. When I finally catch up I will contact you to resume or change my order for future shipments. My question is what information do you need to accomplish cancellation? I enjoy your product very much but use it only on my face so I dont go through it very fast. Im a satisfied customer so by filling out the information below I dont want to be confused with unhappy customers. Thank you for your time.
