I fell in a store sprung my ankle and reinjured my knee that related to back issues. My claim was denied because I waited too long to fill their forms out because I wanted to see my regular physician whom was out on personal leave and knew my health issues. Sad thing is I'm still having problems and due to not having witness info attorney's won't touch it, so I'm screwed. If anyone that reads this got any suggestions please let me know.
