Molina New Mexico SUCKS

My primary care provider scheduled me for physical therapy and shots for my arthritic knee. Twice it was postponed due to paperwork problems with you. The second time I arrived at the physical therapy center I as told the procedure was denied by Molina even though they regularly approve it. I was told to call Molina and inquire why. I reached someone in customer service and they said the procedure was still under review. I contacted the center again and they said they received a definite refusal and could not resubmit for 60 days. I have been trying to contact Molina customer service twice a week for eight weeks using 5 different numbers. Each time I reach customer service I am disconnected! My Member ID is 00312**** - Molina Bronze Plan. My phone number is below. I want to know why the procedure was denied and how to get it reversed immediately. I AM IN PAIN!
