Nurse did vitals &;discussed health issues with patient , b4 doctor came in xam room , SHE LOGED ALL INFORMATION ON SOMEONE ELSES CHART! PATIENT IS A 62 YO FEMALE !! CHART USED WAS A 71 YO MALE !! When doctor came in xam room, she noticed the patient was not a 71 yo male !!! How the *** does this happen !! INCOMPITANCE THERE ARE, ANGRY, HATEFULL ; VENDICTIVE NURSES ARE NOW BEING DOCUMENTED &amp;amp;amp;amp; RECORDED FOR FURTHER REVIEW & RESOLVE !!
