Appealing a deniel of services performed

my husband is a patient at Western Dental in the 1110 South King Address in San Jose cal. Since the middle of December of 2018 me and my daughter called Western Dental called asking if we would need to do anything to continue the coverage for my husband. They told us no that the coverage would be "rolled over" to 2019 and we needn't do anything. He went in on a Saturday January 5. Then on January 12 he went in again. We talked to ALEJANDRA and she called what she said the insurance, I think she mentioned Liberty. She also told me she would place a note on his chart showing my husband did have coverage to the person that would be treating him. Them a few days later I received a letter stating a portion of his procedure was DENIED because of no coverage on the day of the procedure. Then a couple of days later we received a card from LIBERTY stating his coverage being effective since January 1 2019
