Lost money with green dot card

My name is Ernest Lawson. I purchased a green dot card and put $500 on it from Safeway in Davenport WA on May 5, 2021. I was going to activate it today to use the card when I found that the card is missing. Safeway is unable to help me so I called Green Dot customer service and was told that without the card number they cannot help me nor were they able to help me talk to anyone in mgmt or give me corporates phone number so I could speak with someone there. Attached is my information photo copy of the msg Green Dot sent on May 5 the receipt of purchase and photo copies of the information that came with the card.
