Need policy number

I have had this policy for 20 years and the agent rep you have down for me quit, and I can't get a phone number or anywhere to talk to someone on my policy number or how to file a claim form and I want a new rep. I am not happy as long as I have paid into this policy your customer service is very user-friendly
