I called on January 21,2021 , January 27,2021 informing the reps I talked two on two different occasions to get a claim mail to me since I am the beneficiary. The deceased is curtis lee mitchell, sr . Death was 12/4/2020. I was told I should receive my claim by 02/10/21 as of this date I have not received the claim to file for beneficiary benefits my partner left for me. I do not have a correct address.
