This company is committing intentional fraud

I have paid all my premiums for a FL Blue health plan in 2017. FL Blue has sent me a bill with arrears in the range of $3734.36 in March-April 2017. I have been to their office in Tallahassee, FL and also spoke with their customer service people numerous times asking to explain the arrears when all premiums were paid through my bank. They told me they sent a refund check for same amount sometime last year and that amount has to be paid back. I am not sure why the company sent me a check in the first place. I have not received any check for that amount from them. After numerous calls to their incompetent customer service department they finally accepted in December 2017 that the check was never cashed and they would resend the check. I thought matter was resolved. When nothing happened I contacted them again in February. The customer service initially stated they have no record of the conversation in December. Luckily I had the reference number regarding that call. When I provided that number they told me the check was sent and as far as they are concerned the matter is closed even if the check was not cashed. I asked them to provide evidence of the check they sent. They have sent nothing so far in writing. They are not reimbursing the providers for any physician visits or tests who are sending the bills to me. FL Blue has continued to receive monthly premiums from my bank during this entire time. This kind of high handed behavior can only happen in US where companies have a monopoly. Anywhere else in the civilized world the authorities would have investigated the company and taken appropriate action against responsible individuals. I am sure FL Blue have done this to several other customers before me and would continue to do this to others in future. This type of fraudulent practice must be company policy and cannot happen without nod from company CEO or CFO. Clearly company higher ups think they are untouchable. They are taking money from customers and not providing the service they are obligated to. The only option left for me is to approach the courts.
