On January 4, 2019, I filled out the online form for the State of Texas, Texas Vital Statistics department on its official website for 2 death certificates. I ordered and paid online $95.00 and have a record the payment on my credit card went to ZERTIFICA USA. I have never received the certificates. I will dispute the credit payment with my credit card company.
