My PT claim denied

I was on crutches for six weeks. Aflac paid me for my ER visit and for have crutches. I had a broken bone in my knee. AFlac denied my PT claim because it wasn’t started within 30 days. I explained to Aflac my doctor told me not to bare any weight on my foot and to stay in bed. I am now appealing this claim because it is $500.
