Unauthorized cancellation

My client number is CAS-4418030-C9L. I need to request a refund relating to my device that never worked way back since 2014 hence I was paying instalment. My contract was also cancelled without being notified and caused lot of inconvenience. The confirmation of cancellation was over the phone therefore no communication letter.Below is all I have From: Sebothoma, M.
