Truck driver unsafe driving

Driver of truck 243781 on I40 eastbound, 8/07/2019, nearing union cross road, kernersville. Truck exited on Hwy 66 kernersville. Truck driver switched lane in front of me, no signal, too close. I had to hit brakes. I blew horn to warn him I was too close, but came over anyway. I had 6 year old child in car. If I hadn't been observant it would have been a terrible accident. Get him OFF the road. He is dangerous!!
