Store complained for fraudulent event

To whom it may concer, Hope you can read my complaint. It's about the store that we have dine in today ( Aug 4, 2021 at around 12PM PH time). The store is located at Level 1 Robinson Galleria, San Pedro Laguna. We order a group meal that comes with a free supercard. The crew ask to write my name, number and birth date on a piece of paper, for the super card. Note: They are the one who will register my supercard. When the meal was served, pasta was COLD! I complained to the crew, then she immediately re-issued a new hot pasta. As I am busy eating my meal, the crew approach again if I have received a 4 digit code to my phone, I responded "YES". She asked for the 4 digits and said this is for verification purposes. As I continued my lunch, I suddenly received a text message that " You have redeemed your free Birthday pizza & freebie...etc" from SUPERCARD number. I NEVER asked for it to be claimed. I called the crew again to verify and check what happened. She said " Don't mind the text message, it is a doubled transcation" I said, " It is suspicious, why became doubled?" Note: Only my mom and I are the customers at that time. She said the various things that are not acceptable to believe in. It is suspicious and fraudulent on my side. As a remedy, I told her to issue a new card. As I check the Shakeys website, I entered the newly issued card and I registered it. And I saw that the first issued card status is " FREEBIES ARE CLAIMED" on the same day! It is really suspicious why it happened. I am requesting the management to check their staff in that area. And teach them a good habit. If this scenario will continue, a bad reputation will accumulate in that store.
