Malaysia Airlines promised to make full refund but they DID NOT!

Malaysia Airlines failed to issue my ticket due to system error and promised to make full refund to my visa account within 7 to 14 working days. 6 weeks passed, not even a single cent of the refund received. I emailed and called several times to Malaysia Airlines, they had the same answer -- refund had been made. But, why didn't I receive it? I requested Malaysia Airlines to provide any possible evidence but Malaysia Airlines ignored it. This incident has caused me countless of sleepless nights. I wish, by posting this, would help to make more people think twice before buying Malaysia Airlines ticket.
