I accepted and completed a job in 2/2021 and have yet to be paid to date. For me to even have to complete this form is very unnecessary. I completed a contract job and was in line with the posted guidelines and requirements posted at the time of acceptance. My evaluation was not paid out due to a reason I did not expect as an exclusion and since it wasn't listed as a requirement in 2/21 then payment for my time and services should be made.
