I have not received my W2

My name is Yaphet Graham and I previously worked for the Beatties Ford Rd location in Charlotte, NC. I have yet to receive my W2 and I cannot get the general manager to answer his personal or work numbers. Can someone please help me attain my W2 so I do not incur any unnecessary penalties from the state or federal government?
