Dear Sir/Madam, I am insured with Desjardins via my employer under your Group Insurance. I am hoping that you or someone within your company can assist me in obtaining a decision letter. A decision was made to deny my LTD benefits; however no explanation was provided, via email or posted mail. I was diagnosed with the following: - Seronegative Inflammatory Arthritis - 5 bulging disc in my lower Spine - PTSD - bi-polar - Stress and anxiety issues - Daily chronic pain in my head, neck, both eyes, fore-head, shoulder, upper back and bi-lateral sciatic pain - dizziness - tingling in my bi-lateral fingers and toes - Insomnia - Regularly vomiting after meals - Severe and chronic photo-sensitivity in bi-lateral eyes - Diagnosed having a concussion during my accident - Renal colic As of today May 17, 2018 I have not received a decision letter as to the reason/s for my LTD denial. My claim for LTD started in the Fall of 2017. I was told in early January 2018 I am denied LTD via email. NO EXPLANATION PROVIDED. I would like to request a complete copy of my entire file. I need to have the Ontario Ombudsman's office look into this as well as FSCO. Thanks, Bobby
