Cannot cancel this service no matter how hard I try

I cancelled in Nov. 2017. The customer service person said they would email me a document to send back. Never got an email and forgot - thought it was cancelled. Just noticed that charges continued to come out of the bank. Called back and they said they have a record of my cancellation. Turns out they emailed the document I have to sign and return to the wrong email address. I made them spell back the address this time and they got it wrong again. Now I'm waiting for the email with my magical cancellation document that I must send back to really cancel my account. Meanwhile I've called my bank and stopped payment to Monitronics.
