Cannot get you to cancel my suscription! Please HELP!!

I have contacted 6mos. ago and cancelled my subscription, I didn't hear anything further and assumed it had been cancelled. Now, I received a card telling me I will be billed for 12 months of Private Briefing. I do not want this and cancel anything in my account. ACC#00004202**** REFERENCE#00004495****. Please do not charge my account for $99.00
