Why is my Claim being denied?

On 06/26/00 I injured my left knee at work for Zurich Insurance. For 19 years my claim has been paid. Today I was told my Claim has been denied. I am having surgery on 06/21/19 for a Total Knee Replacement. What is going on? Someone needs to contact me @ 412-247-****. If this is not resolved, I will be seeking the services of an attorney. My Claim # 005380-00****-wc-01.
