Unprofessional approach to helping a patient

My wife and I went for a second visit to Dr. Silk the eye doctor. He checked her eye. We asked if he could help sign a paper so my wife can get help with transportation using a taxi to help her get around. First he said there would be a fee then asked me to pay a bill that is not due until 9/26/15. This was for a July visit but we didn't receive the bill until late August. So they said they won't send this document in until I pay their $ 30 fee and also the bill. I told them this is like extortion. I thought they were in the business of helping people.
