Unable to discover why I received a bill

I received a flu shot at Minute Clinic, and the nurse was wonderful. They took my insurance information. Over a month later I received an email notice that I had a bill without any information about how much the bill was or what it was for. I have tried for several days to use the billing number on the website to obtain information about the bill. After following the directions on the telephone, the call is disconnected. I went to CVS for information, but no one at the Minute Clinic could provide information. I am beyond frustrated! I am not going to pay a bill of unknown cost for unknown service! User's recommendation: Obtain your flu shot elsewhere!
