Requested a simple invoice-statement (screw up)

first time I requested a statement-invoice with medicine, quantity, dosage amount, and NDC along with claim authorization they were able to provide it in 2 business days. I submit my copay of $250 to be reimbursed. Recently, a drug filled on Aug 14 but the request for my statement-invoice has been aggravating since it is now in the 5th week of request. I submitted for an invoice-statement on Sept 5. After nearly a dozen phone calls 6 secure email msg but only 2 documentations--they still screwed it up. This is coming from accounts receivable dept. How bad and inconsistent is BriovaRx?
