Refuse to approve a preauthorized hospital stay

This insurance refuses to approve a hospital stay that it preauthorized by saying my insurance was not active which is totally inaccurate. I have already paid my deductible but it is not reflected because they have not approved the claim after three months and many appeals. My next stop is the Texas Insurance Commission.
