Rebate forms missing/lost

Purchased four tires from TireRack.com on Oct 27th, 2014. Completed Rebate form on November 5th, 2014 sent out next day with original receipt. After six weeks Checked status on MyrebateOnline, no information found. Called the rebate # [protected], CSR said that information wasn't recieved. CSR said to contact HANKOOK by going to hankooktireusa.com. I made records of everything. Still denying my rebate. Too much wasted effort getting $40 back. Will never buy from Hankook or Tirerack again.
