Not doing their job

I have called more than once along with my spouse to get a form faxed and emailed to get passed sent money documentation proof for a fraud case and they seem to not care or send the form in the time frame they state. This has been 3 days now since our first attempt. A total of 4 phone calls and one office location attempt. Would like this taken care of asap.
