Need forms to ask for state owed arrears waived.

I have been diagnosed with a mental illness which takes a lot out of me to maintain. I am on SSI which I have been on for a while, I feel i am not able to pay this debt because I only get enough to barely get by.ive tried to work but it always takes a toll on me.What do I do?
