Lid from pan exploded in face

I was cooking hamburger in pan. It had been in the pan 10 minutes maybe, then i heard a very loud pop and glass flew everywhere. I'm extremely lucky it missed my eyes. All food had to be thrown out. Pan lid was in small squares of glass everywhere. User's recommendation: Send a warning with cookware warning against explosion.
