Great until you have to cancel

Service was fairly good the entire time. Internet was decently fast and reliable and had dish service bundled. The problem arose when I had to cancel. I cancelled in may and turned in all equipment. Three months later I am now getting a bill for my dish DVR. (Which I turned in). It is next to impossible to find out who to talk to about the issue. I called dish and they say call windstream and windstream says call dish. In the meantime the windstream associate is incredibly rude and hung up when I requested a supervisor. So now I am stuck with a bill WHEN I DON'T EVEN HAVE THE EQUIPMENT IN USE.
