I went in to use the bathroom at the Biddeford, Maine Cumberland Farms on Alfred Street on 1-25-21 the bathroom floor had been mopped the wet floor sign was out against the wall I went in and went to the bathroom went to the sink and turned around to go dry my hands and never made it slipped and fell face first onto the floor nobody came in no one heard the trash can fall or anything I got up went out and told the store employee said are you okay I said I don't know so far it seems like I am I can't tell you. Then I called the customer service number and I left them know, and they made a claim and filed a claim on it, I was supposed to go on last Monday the 1st of February to the orthopedic surgeon but because I had no way there I had no car my car had gotten stolen, and I had no transportation I had to change it to see the orthopedic surgeon on February 10th so as soon as I know from them if this fall is made the hip worse I will let you guys know and the claim can go forward, and we can see what we can do work out here cuz I'm out of work due to this and if I have to have hip surgery I'll be out for three to six months because of it I was already do to have surgery eventually but the following me happen kind of quicker if you have any questions you can call me I don't know if my phone will still be on because I have no money to pay my cell phone, so it may get shut off at 802-3****. I am in severe pain it seems to be getting worse and worse each day, but I'm not doing anything I have to stay home I can't really do anything besides set and lay in bed I can't lift anything can't do a lot of bending or anything I'm on pain medicine for nighttime so when I sleep I can sleep because it's very painful at nighttime I did make a report like I said to the customer service when I call every time though it doesn't let me get through to them sometimes it does but sometimes it doesn't but my name is Jeffrey Danis.
