Hi ! I was a member in 2019 and I recieved a bill from quest. I would like a EOB please. My insurance ID is LFSC731215. My name is Nadine Francoise Tekdemir. Birthdate is 11/15/1992. I've tried calling customer service but I cant get through because I do not have my member ID card anymore. Please call me at 631 336**** or email nplacide2009@***.com.
