Didn’t receive a W-2

I worked for the company in October 2018 for a few months. I had to leave the company because they had me driving all over GA and SC 7 days a week from 6 am to 10 pm. I've requested my W2 several times, but I keep getting the runarounds. Can someone please help me because I need to file my taxes so I can take care of my kids?
