The Dentist Donald P. Jonker, D.D.S doing business at Family Dental Care, Elite Dental Partners, Bloomingdale, IL., and his office staff: (1) Failed to perform the approved dental services despite pre-checking and pre-authorization with my dental insurance, the Met Life Insurance company (2) They failed to pre-authorize excessive charges for uncovered procedure/s. (3) They failed to disclose the hidden charges to be borne by me, the patient (4) They failed to submit charges in time with the insurance company and subsequently end up in less balance for total benefit available when notified after more than a year. (5) Their dental office has a FALSE ADVERTISEMENT, exceeding the limits of Truth in advertisement (6) They performed unprofessional services of Bait and Switch when specials on advertisement were published for attracting new patients (7) They charged excessively for the services due to incompetence of the doctor and staff to provide required services within reasonable prudence, where there was no need to perform surgical procedure (8) They did not follow upon the complaints of pain and suffering after the unwarranted procedures. (9) They failed to address the legitimate concerns of pain and suffering and chipped tooth during the extraction tooth surgery.
