Daughter not granted anesthesia coverage

oral surgeon "pre-authorized" removal of 12 yr old daughter's 4 baby teeth with Aetna. 5 months later we are getting a bill for $400 for anesthesia. Aetna claims for 4 teeth they won't grant anesthesia but they will if it is "5 or more teeth being removed." According to oral surgeon Aetna updated it's policy. What the *** My daughter was in considerable pain as it was without their suggestion that she be completely awake through the procedure. *** Cruel and unusual to say the least.
