i was hospitalized july 19-21, 2010 at paul b hall in paintsville ky. i was assigned kitta kousa, as i did not have a family doctor. she never discussed any test with me, nor would she sign a release for me on july 21, even though she had said i could go home that day. i signed myself out of the hospital at 6:30 pm, when she would not respond to several calls to her from the staff. also, i have aflac, and no other health insurance. i sent a form to her office with a self addressed stamped envelope, for her to complete. her office called and said i had to come into her office, and pay a office visit fee in order for her to give me the form. i do not have health insurance and can not pay an office visit fee. i will not ever have her again if i am hospitalized, even though i loved the care that paul b hall gave me sincerely, janet borders
