Cannot Redeem cash reward I legitimately earned.

Said I needed to verify my identity. No problems with prior redemptions. Multiple requests to be placed in their call queue to resolve issue have been unanswered. They requested a completed W9 which was sent via FedEx and I received confirmation of delivery. Have not received postcard with verification code. Any ideas on how to resolve and get my cash? User's recommendation: Beware of Greeks bearing gifts.
