Cancellation of subscription return last prescription

Good afternoon Provia. First of all , your product did not work for me. No problem. However, soon after, I tried to cancel the prescription and could not get any direct or email working contact. Before I could get any contact, which I still haven't other than this, you sent and charged me for another. Return label request and a reimbursement needed please.
